We discuss evolving traditional care management programs of Medicaid managed care health plans, or managed care organizations (MCOs). This paper:

• Explores some of the key drivers for the changes in the Medicaid care management space
• Addresses the challenges in transitioning to new models of care
• Proposes approaches that Medicaid MCOs can take to implement and optimize new care models
